WSIB Form 3947A: Provider Payment Request

Health care providers and clinics that do not bill the WSIB electronically through TELUS Health can request payment for services on this form. Each line takes one WSIB service code with its fee, the number of services and the dates given, and the lines add up to a total billed. It is not for medical reports, which are billed on the WSIB report form itself.

Who completes it

The provider or clinic completes the whole form: the worker's claim number and details, the provider's 9-digit WSIB Provider ID, HST and invoice numbers, the service lines, and a certification, signed by the person completing it, that the information is true, correct and complete.

Submitting and payment

How to submit it
Submit it with any supporting documents through the WSIB's online submission page, which now requires a WSIB online services login. For invoices, the WSIB points providers first to electronic billing through TELUS Health.
Who pays
The WSIB pays the fees in its Fee Schedule for each service code billed, with any HST claimed under service code ONHST, once it has the claim number and Provider ID it needs to process payment.

Good to know

  • Do not use this form to bill for medical reports: complete the billing section of the pre-printed WSIB report form, or put a payment label on the front of a narrative report.
  • Use a separate line for each month and service code, tick the dates of service, and leave out services you have already billed.
  • The claim number and 9-digit WSIB Provider ID are required to process payment, and incomplete or illegible forms create processing delays.
  • Equipment and supplies go on the sister form, Provider payment request for equipment/supplies (3941A); a checkbox lets you sign 3947A electronically with your typed name and date.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Can I bill the WSIB for a medical report on form 3947A?

No. The WSIB says not to use the Provider Payment Request to bill for medical reports. Bill a report through the billing section on the pre-printed WSIB report form, or place a payment label on the front page, bottom right corner, of a narrative report.

How do I fill in the WSIB provider payment request?

Enter the worker's claim number, name, date of birth, date of incident and, if available, the diagnosis or ICD 9 code, then your WSIB Provider ID and invoice number. Use one line per service code and month, with the fee from the WSIB Fee Schedule, the number of services and the dates, and sign the certification.

Should I use form 3947A or TELUS Health?

The WSIB directs providers to TELUS Health to submit invoices and to register to bill and get paid online. Form 3947A is the alternative, submitted through the WSIB's online submission page with an online services login. Payment requests for equipment and supplies use the separate form 3941A.
