WSIB MSK Program of Care Initial Assessment (10635A)

Regulated health professionals delivering the WSIB musculoskeletal program of care complete this six-page report after assessing a person at the start of block one. It records the injury history, examination findings, occupational diagnosis, recovery barriers, a functional outcome measure, the block one treatment plan and abilities and restrictions for return-to-work planning. Since May 1, 2023, community-based MSK treatment for WSIB claims must be delivered through this program.

Who completes it

The registered regulated health professional delivering the program, such as a physiotherapist or chiropractor, completes and signs the whole report, including the abilities and restrictions pages.

Submitting and payment

How to submit it
Submit it online through WSIB online services at wsib.ca/submit within two business days of the assessment, and bill it electronically through TELUS Health.
Who pays
Bill the report under MPOCIAF and block one under MPOCSB1 (single zone) or MPOCMB1 (multiple zones); block one payment is withheld until the WSIB receives both the initial assessment and mid-point reports and their billing.

Good to know

  • The program is for people within eight weeks of their workplace injury; block one is pre-approved for a single zone, while treating multiple zones needs WSIB approval.
  • Block one requires at least five visits for the full block fee; with fewer visits the fee is reduced by 50 per cent.
  • Complete at least one functional outcome measure for the injured area (NDI, ODI, QuickDASH, LEFS or WHODAS) and repeat the same measure throughout treatment.
  • The program replaced the WSIB's former low back, shoulder and musculoskeletal programs of care, and each regulated health professional delivering it must register, at every clinic location.
