ODSP Form 3261E: Invoice for DDP and Medical Review
Health professionals who are not paid through OHIP use this ministry invoice to bill for completing an ODSP Disability Determination Package or Medical Review Package, or for Additional Medical Information requested by the Disability Adjudication Unit. It lists the payee, the client, the services rendered and the totals. Physicians use it only for requested Additional Medical Information, since they bill OHIP directly for the packages.
Who completes it
The professional who completed the forms fills in the whole invoice: psychologists, optometrists, occupational therapists, physiotherapists, social workers and chiropractors, for example. It needs your full name, profession and contact details, the client's full name, date of birth and social assistance member ID, and the sections you completed.
Submitting and payment

- Mail the invoice to the ODSP Disability Adjudication Unit. The fillable PDF opens only in Adobe Acrobat or a current Adobe Reader, not in a browser or in older Reader versions such as XI.
- Who pays
- The Ministry of Children, Community and Social Services pays the set fee for each section completed, plus HST where applicable, while physicians bill OHIP fee codes such as K050 to K052 and K057 to K060 instead.
Good to know
- Say which sections you completed: the Health Status Report, the Activities of Daily Living Index, or both, since each carries its own fee.
- Physicians bill OHIP directly: K050, K051 or K052 for the Disability Determination Package, and K057 to K060 for the Medical Review Package parts.
- Additional Medical Information is due back to the Disability Adjudication Unit within 45 days of the request; the Disability Determination Package has 90 calendar days.

What OHIP codes do physicians use for ODSP disability forms?
For the Disability Determination Package, K050 covers the Health Status Report and Activities of Daily Living Index together, K051 the HSR only and K052 the ADL only. For the Medical Review Package, K057 covers Part A, and K058, K059 and K060 cover Part B: both parts, HSR only and ADL only.
Why won't the ODSP invoice form open?
Form 3261 is built for Adobe Reader. Ontario's forms repository says older versions, including Reader XI, will not open these forms, and browsers and most PDF tools cannot display its fields. Download the PDF and open it in the latest free Adobe Reader to fill it in and print it.
