OTPP Form 0202: Medical Report, Disability Pension

Ontario Teachers' Pension Plan members use this report to seek medical pre-approval for a full or partial disability pension, or to apply for one. OTPP's independent medical referee reviews it. Eligibility is based on the member's condition when they resigned from employment in education, so the medical examiner is asked for as much detail as possible about that point in time.

Who completes it

The applicant completes Part 1 (personal details, doctors consulted, medical history and a description of the disability) and signs the Part 3 authorization. The medical examiner, the treating physician, completes Part 2.

Submitting and payment

How to submit it
The applicant sends the completed, signed form to OTPP by mail or fax, with a full set of supporting medical reports from the doctor or specialist, as soon as possible.
Who pays
Fees for the examination and for completing the report are the patient's responsibility.

Good to know

  • Once medically pre-approved, the member has one year to apply formally, provided they are still within two years of their resignation date.
  • Any worsening of the condition after the member's termination of employment in education is not taken into account when OTPP decides eligibility.
  • Late applications are accepted only if a late diagnosis or the disability itself prevented the member from applying on time.
  • Part 2 asks whether the patient can teach or perform their previous duties, work in another capacity, or ever return to education, and whether disclosure would prejudice their health.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Why does OTPP recommend medical pre-approval?

Members must give up LTIP or WSIB loss of earnings benefits and sever their employment before a disability pension starts, and once approved they can no longer transfer the commuted value of their pension. Pre-approval lets them learn whether their condition meets the plan's requirements before making those choices.

What is the difference between a full and partial OTPP disability pension?

On the form, partial means the member cannot work in education but other employment is possible, or their health may improve. Full means they are unable to work at any job at this time. The independent medical referee assesses which, if either, applies.

What should the doctor attach to the OTPP medical report?

Copies of any documents and reports from other doctors or specialists. OTPP asks members to submit a full and complete set of supporting medical reports with the form, and the examiner should list diagnoses in order of significance and explain if details at resignation are unavailable.
