OPB 3003: PSPP Medical Examination Report

Ontario Public Service Pension Plan members applying for disability benefits, or asked by the Ontario Pension Board to have their condition reassessed, need this physician report. The member sends it to the physician with a copy of their job description and the accompanying Physical Demands Analysis. Much of the report measures the condition against the essential duties in those documents.

Who completes it

The member completes page 1, including employer, position, last date of work and a signed authorization for OPB's medical consultants. The physician completes pages 2 to 6, answering all sections and striking out those that do not apply.

Submitting and payment

How to submit it
The physician returns the completed report to the applicant, who files it with the Ontario Pension Board by mail or fax.
Who pays
The applicant is responsible for paying any fees the physician charges for completing the report.

Frequently asked questions

What should the physician review before completing OPB 3003?

The member's job description and the accompanying Physical Demands Analysis. The form tells physicians to review both before completing the report and refers to the Physical Demands Analysis for the essential duties in sections 6, 7 and 8. It also asks for precise details to help the applicant.

Is OPB 3003 used for reassessments?

Yes. The physician instructions say the applicant is either applying for disability benefits from the Public Service Pension Plan or has been asked to have their medical condition reassessed. The same six-page report, current version dated 2020-06, is used in both situations.

Who can sign the PSPP Medical Examination Report?

A physician or medical professional recognized by the appropriate governing medical association in Canada or the USA, such as the Canadian Medical Association or American Medical Association. The signature block also asks whether the signer is a certified specialist, and if so, in which specialty.
