OMERS Form 147: Medical Report for Total Disability

OMERS members use this three-page report to give OMERS new or updated medical information for a total disability benefit. OMERS uses it to decide whether the member qualifies, or still qualifies, for a disability waiver of contributions or a disability pension. Because OMERS conducts regular reviews, the same form is also used to send updated medical information when OMERS asks for it.

Who completes it

The member completes Section 1 (member information) and, if not already submitted, Section 2 (member election). The member's doctor, licensed in a Canadian province or where the member lives, completes Section 3 (medical information).

Submitting and payment

How to submit it
The member sends the completed form to OMERS, which recommends uploading it through secure communications in the member's myOMERS account.
Who pays
OMERS states that it is not responsible for any costs of completing this form or of providing medical documents to OMERS.

Good to know

  • For the first 24 months, the test is incapacity that wholly prevents the regular duties of the member's own occupation; afterwards, any work suited to their education, training or experience.
  • The disability pension question adds a lifetime test, and the doctor must explain why the member is expected to be unable to do any paid work for life.
  • OMERS accepts copies of medical forms or reports the doctor completed for other benefits; in that case the doctor does not need to complete Section 3.
  • Section 3 asks for the date of total disability, the date it affected work, diagnosis, subjective symptoms, objective findings such as x-rays or tests, and prognosis.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

What does the doctor need to answer on OMERS Form 147?

After the diagnosis, symptoms, findings and prognosis, the doctor answers three yes/no tests: restrictions preventing full regular duties, restrictions preventing any work, and whether the member meets the lifetime disability pension definition. Restrictions need associated timelines, plus estimated dates for returning to full regular duties or to any work.

Can a doctor send an existing report instead of Form 147?

Yes. OMERS says it will accept copies of medical forms or reports about the member's condition that the doctor has completed for other benefits, and the doctor then does not need to complete Section 3. The member still completes Sections 1 and 2 as required.

Does OMERS contact the employer about a disability claim?

It may. The form says OMERS may contact the employer to obtain outstanding information about the member's leave period. Section 2, the member election, is required for any disability claim the employer reports, unless the member has already signed and submitted the election form.
