Frequently asked questions

Can a physician apply for a child's emergency admission to secure treatment in Ontario?

Yes, but only for a child who is 16 or older and under 18. Form ON00325E lists the child, the child's parent, a Children's Aid Society officer with custody, or a physician as applicants in that age group; for younger children, a physician is not on the list.

Is ON00325E available in French?

Yes. The Central Forms Repository posts the French version, Demande de placement d'urgence dans un programme de traitement en milieu fermé (ON00325F), alongside the English form. The current English edition is ON00325E (2022/12), a fillable PDF.

What does the child's consent section require?

When the criterion in clause 171(2)(b) of the Act is not met, the child signs a statement that they have had legal advice and consent to the emergency admission. If the child is under 16, a parent or the Children's Aid Society with lawful custody must also consent.
