Ontario Hospital Certificate of Death: Form 1 (0005-54)

Ontario hospitals use the Certificate of Death, Form 1 under Regulation 965 (Hospital Management) of the Public Hospitals Act, when a patient dies in hospital, before the body is released. The attending physician or registered nurse in the extended class records the patient's name, the date and hour of death, and the cause of death in two parts, with the approximate interval between onset and death.

Who completes it

The attending physician or registered nurse in the extended class completes and signs the whole certificate, as the Central Forms Repository description states.

Submitting and payment

How to submit it
The certificate is completed in hospital before the body is released, under section 17(2)(a) of Regulation 965; registering the death uses the separate Medical Certificate of Death, Form 16, under the Vital Statistics Act.

Good to know

  • Part I records the immediate cause, meaning the disease, injury or complication that caused death, not the mode of dying such as heart failure, asphyxia or asthenia.
  • Below the immediate cause, list the morbid conditions that gave rise to it, in order backwards, on the due to lines (b) and (c).
  • Part II records other important morbid conditions that contributed to the death but are not causally related to the immediate cause.
  • The fillable PDF is an XFA form that opens correctly only in Adobe Acrobat or Reader, and a French version (0005-54F) is posted alongside it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can sign the Ontario hospital Certificate of Death?

The attending physician or a registered nurse in the extended class. The Central Forms Repository describes Form 1 as completed by an attending physician or RN(EC) under section 17(2)(a) of Regulation 965, Hospital Management, made under the Public Hospitals Act.

Is the hospital Certificate of Death the same as Form 16?

No. Form 1 is the hospital's certificate under the Public Hospitals Act, completed before the body is released. The Medical Certificate of Death, Form 16, is the Vital Statistics Act form delivered to the funeral director and used to register the death and issue a burial permit.

How should the cause of death be written on Form 1?

State the disease, injury or complication that caused death as the immediate cause, not the mode of dying, then list the conditions that led to it in order backwards. Give the approximate interval between onset and death for each, and add other contributing conditions in Part II.
