Ontario Child Care Medical Exemption Form (3041E)

Ontario's Ministry of Education uses form 3041E when a child, or an individual, at a licensed child care centre or home child care agency needs a medical exemption from the immunization requirements of O. Reg. 137/15 under the Child Care and Early Years Act, 2014. The health professional marks the reason and length of exemption for each disease, and the statement goes to the child care provider.

Who completes it

A regulated health professional completes the Section 2 declaration and signs Section 3 with a registration or licence number; the declaration heading also names a physician or registered nurse (nurse practitioner). The parent, or the individual, fills in Section 1 and names the child care centre or home child care agency.

Submitting and payment

How to submit it
The completed statement is given to the licensed child care centre or home child care agency named in Section 1. The Ministry of Education and the Medical Officer of Health may collect the information from the provider.

Good to know

  • The form lists diphtheria, tetanus, pertussis, polio, meningococcal disease, measles, mumps, rubella, Hib and varicella, each with immunity or contraindication reasons and a permanent or temporary exemption.
  • Immunity can be shown by clinical diagnosis of prior disease or laboratory confirmation; for varicella, a clinical diagnosis of prior varicella or herpes zoster is acceptable.
  • Temporary exemptions need from and to dates, and contraindications detrimental to health are explained in a free-text box on the form.
  • One file holds two versions: one for a child, under subsection 35(2) of O. Reg. 137/15, and one for an individual, under subsection 57(3).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can sign Ontario's child care medical exemption form?

A regulated health professional signs Section 3 and gives a registration or licence number. The Section 2 heading names a physician or registered nurse (nurse practitioner), and the declaration certifies that, for the medical reasons indicated, the child or individual should be exempted from the requirements of O. Reg. 137/15.

Is form 3041E the same as the school medical exemption?

No. Form 3041E is the Ministry of Education's statement for licensed child care under the Child Care and Early Years Act, 2014. School immunization exemptions use a separate Ministry of Health form, 4895-64E, under the Immunization of School Pupils Act, which does not list Hib.

Does form 3041E open in a web browser?

No. It is an XFA-based fillable PDF that opens and fills correctly only in Adobe Acrobat or Reader, and the Central Forms Repository warns that older versions such as Adobe Reader XI no longer work. A French version, 3041F, is posted alongside it.
