Form 3089E: Birth Family Severe Medical Search (ON)

Birth parents and other birth relatives in Ontario use this application to ask the Custodian of Adoption Information to locate an adopted person, or their descendants, so that medical information can be obtained or shared. It applies when that information would significantly increase the likelihood of diagnosing or treating a severe mental or physical illness. A health care professional's questionnaire supplies the medical justification.

Who completes it

The birth family applicant completes Parts A to C and signs the statement in Part E. A physician or other regulated health care professional completes the Part D questionnaire after the patient signs its consent to disclose health information.

Submitting and payment

How to submit it
The applicant mails the application, including the signed health care professional questionnaire, to the Custodian of Adoption Information. Incomplete or incorrect applications are returned and processing is delayed.

Good to know

  • The guide asks the health care professional to verify the nature, severity and urgency of the situation, and to confirm professional status with a business card or letterhead.
  • Part D asks whether there is a genetic reason to seek or pass on the information and whether denying the search would have adverse health effects.
  • Descendants of an adopted person cannot use this form; adopted people and their descendants apply with form 3090E, which uses the same professional questionnaire.
  • Requests that do not meet the eligibility criteria are not granted, and the custodian may contact the health care professional directly about the application.

The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.
