Ontario Form 3090E: Severe Medical Search (Adoptee)

Adopted people and their descendants in Ontario use this application to ask the Custodian of Adoption Information to locate birth relatives when medical information could significantly improve the diagnosis or treatment of a severe mental or physical illness. The search can obtain information, share it, or both. Part D, the health care professional questionnaire, supplies the medical justification the custodian relies on to decide eligibility.

Who completes it

The applicant completes Parts A to C and F, plus Part E consent from an adoptive parent or guardian if under 18. A physician or other regulated health care professional completes Part D after the patient signs its consent to disclose health information.

Submitting and payment

How to submit it
The applicant mails the full application, including the health care professional questionnaire, to the Custodian of Adoption Information. Applications without a completed, signed questionnaire are returned.

Good to know

  • Part D asks for the presenting problem, diagnosis and prognosis, whether the information is essential to diagnosis or treatment, any genetic reason, and the adverse effects of refusing the search.
  • Confirm your professional status with a stamp, business card or letterhead; the guide says the Custodian of Adoption Information may contact you about the application.
  • The custodian can only search for adoptions registered in Ontario, and incomplete applications are returned, which delays processing.
  • Birth family members cannot use this form; they apply with form 3089E, which contains the same health care professional questionnaire.

How this list is checked

The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.
