Ontario's Assistive Devices Program uses this form to fund a real-time continuous glucose monitor for people with type 1 diabetes who meet set hypoglycemia-risk criteria, with one form for adults and children. A specialist physician or nurse practitioner linked to an ADP-registered Diabetes Education Program confirms eligibility and signs. ADP then covers sensors and transmitters in full, paid to the vendor, up to a maximum quantity per 24 months.
Section 2 (Eligibility) must be completed by an endocrinologist, another specialist physician or a nurse practitioner associated with a diabetes program, who also signs Section 4 and names the Diabetes Education Program and its ADP clinic number. The applicant or agent completes Sections 1 and 3, and the ADP-registered vendor completes the vendor block.

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

The Section 4 prescriber must be a physician or nurse practitioner certifying that the applicant has type 1 diabetes and meets the Section 2 criteria. The form specifies an endocrinologist, another specialist physician, or a nurse practitioner associated with one of the diabetes programs registered with ADP.
Yes. If the insulin pump uses rtCGM technology, the patient may be eligible for ADP coverage of the sensors and transmitters. According to the applicant information sheet, they still need to meet the device-specific rtCGM medical criteria and submit this application form.
