Ontario IMD Program Application and Consent (4551-87E)

Patients diagnosed with an eligible metabolic disorder can receive funded products through Ontario's Inherited Metabolic Diseases (IMD) Program, and this three-page form is the application and consent to join it. A treating physician at one of six designated hospitals completes the clinical sections, and the patient or substitute decision-maker signs the declaration. The ministry then records its approval of the registration.

Who completes it

A physician, usually the treating physician named in Section 2, completes the application and must sign Section 5 with a CPSO number; the patient or their substitute decision-maker signs the declaration and consent on the last page.

Submitting and payment

How to submit it
Submit through the ministry's SADIE portal for faster approval, or fax or mail the form to the Ministry of Health's Delivery and Eligibility Review Branch.

Good to know

  • The patient must be treated by a physician at CHEO, the Hospital for Sick Children, Kingston Health Sciences Centre, London Health Sciences Centre, McMaster Children's Hospital or University Health Network.
  • Patients with private insurance or employer benefits that cover the needed IMD products are not eligible, and the declaration requires notifying the ministry of any coverage change.
  • The diagnosis must come from the program's approved list of metabolic disorders, with the date of diagnosis; a separate section records the patient's dietitian.
  • The PDF is an XFA e-form that fills correctly only in Adobe Acrobat or Reader; the current version is 4551-87E (2025/04), and a French version exists.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Who can sign the Ontario IMD Program application?

Section 5 requires a physician signature with CPSO number. If the physician completing the application is not the treating physician named in Section 2, they add their own address and contact details. The patient, or a substitute decision-maker with authority, signs the declaration and consent on page three.

Which hospitals qualify for Ontario's IMD Program?

The patient must be treated by a physician at the Children's Hospital of Eastern Ontario, the Hospital for Sick Children, Kingston Health Sciences Centre, London Health Sciences Centre, McMaster Children's Hospital or the University Health Network, according to the eligibility rules printed on form 4551-87E.

How do I submit the IMD Program application?

The ministry recommends the SADIE portal for faster approval. Alternatively, fax or mail the completed form to the Delivery and Eligibility Review Branch of the Ministry of Health. Because it is an XFA form, open it in Adobe Acrobat or Reader rather than a browser to fill it in.
