Ontario Nutrition Products Form 3057-87 (ODB)

The Ontario Drug Benefit program reimburses nutrition products only when they are the patient's sole source of nutrition and an eligibility criterion is met, such as dysphagia, malabsorption or a chemically defined diet for Crohn's disease. This bilingual two-page form is the prescriber's authorization, and pharmacies must hold a fully completed copy before billing. It stays valid for one year from the date the prescriber completes it.

Who completes it

The prescriber completes all of Section 1, which pharmacists are not allowed to fill in on their behalf; the pharmacist completes Section 2 before the first dispensing.

Submitting and payment

How to submit it
Give the completed form to the patient with the prescription, or fax it directly to the patient's pharmacy, which keeps it on file for two years.

Good to know

  • Exclusions include weight loss for obesity, food allergies, body building, voluntary meal replacement, nutritional supplements, convenience, and replacing breastfeeding for infants with normal gastrointestinal absorption.
  • Patients who tolerate some solid foods and need only supplementation are not eligible, and prescribers should not complete the form if the criteria are not met.
  • The ministry audits pharmacies; a form that is incomplete or leaves doubt about the criteria leads to recovery of funds from the pharmacy.
  • Part IX of the ODB Formulary lists the eligible nutrition products and criteria, which are also printed on the back of the form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

When does ODB cover nutrition products?

When they are prescribed as the patient's sole source of nutrition and the patient has one of three conditions: oropharyngeal or gastrointestinal disorders causing dysphagia, maldigestion or malabsorption with significant gut failure, or a need for a chemically defined diet as primary treatment, such as in Crohn's disease.

Can a pharmacist complete the Nutrition Products form?

Only Section 2. The form states pharmacists cannot complete Section 1 on the prescriber's behalf. Forms missing Section 1 information go back to the prescriber for completion, or the prescriber faxes the pharmacy a revised form before the prescription is filled.

How long is the Ontario Nutrition Products form valid?

One year from the date the prescriber completes it. Each ODB claim for a nutrition product must be supported by a valid, fully completed form, so a new form is needed once the year has passed. Pharmacies keep a copy on file for two years.
