Ontario Special Authorization (Allergen) 2772-87E

Ontario's Ministry of Health funds certain allergen extracts for people eligible for the Ontario Drug Benefit program, and this form must be completed for the extract to be funded. The prescriber authorizes one prescription, describing the extract's contents and concentration, and forwards the form to the allergen extract supplier or pharmacy. The dispensary then completes its section and submits the claim.

Who completes it

The prescriber, a physician registered with the CPSO or a nurse practitioner registered with the CNO, completes and signs Section A; the dispensary completes Section B with the claim details.

Submitting and payment

How to submit it
After completing Section A, forward the form to the allergen extract supplier or pharmacy; the dispensary sends the completed claim to the Ministry of Health's Claims Services Branch by fax.

Good to know

  • The form covers one prescription for the extract described and any authorized refills, for up to two years from the prescriber's signature date.
  • A new form is needed for any allergen extract not described on the existing form or prescribed by a different prescriber.
  • Prescriber identification uses the regulatory college registration number with reference code 01 for CPSO or 44 for CNO; mandatory fields are marked with an asterisk.
  • It is an XFA e-form that fills correctly only in Adobe Acrobat or Reader; a French version, Autorisation spéciale (allergène) 2772-87F, is available.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who completes Ontario's Special Authorization (Allergen) form?

The prescriber completes Section A: prescriber name and college registration number with the CPSO or CNO code, patient name, date of birth and ODB eligibility or health number, the contents and concentration of the extract, and a signature. The dispensary completes Section B when billing.

How long is an Ontario allergen special authorization valid?

Up to two years from the date the prescriber signs it, covering the one prescription for the allergen extract described and any refills authorized under that prescription. A new form is required for a different extract or when another prescriber writes the prescription.

Where does the allergen form go after the prescriber signs it?

The prescriber forwards it to the allergen extract supplier or pharmacy. The dispensary completes Section B, certifies that the costs reflect the actual acquisition cost with only the eligible mark-up, and submits the claim to the Ministry of Health's Claims Services Branch.
