Ontario Commercial Driver Medical Report (SR-LC-080)

Ontario requires this report from applicants for and holders of class A, B, C, D, E and F licences: every five years under age 46, every three years from 46 to 64 and every year from 65. MTO mails the form to the driver 90 days before it is due, and a late report downgrades the licence to class G.

Who completes it

The driver completes Part 1, including the certification and release of information; a nurse practitioner or physician completes Part 2, Patient Health Information, and Part 3, noting whether they provide continuing or episodic care.

Submitting and payment

How to submit it
The form lists Medical Review Online (DriverPortal), online fax, email or mail to MTO's Driver Medical Review Office; ontario.ca adds eForms at some clinics and in-person drop-off at DriveTest when questions 2 to 9 are all 'no'.
Who pays
The form states the cost is the patient's responsibility and is not a benefit of OHIP or the Ministry of Transportation.

Good to know

  • The vision section must always be completed and asks whether the driver meets the commercial standard: no poorer than 20/30 with both eyes and 20/100 in the weaker eye.
  • Part 2 also covers cardiovascular disease (NYHA class, ejection fraction), diabetes and severe hypoglycemia in the past 6 months, seizures, sleep disorders, cognition with an MMSE or MoCA score, and hearing.
  • A satisfactory report filed within one year of the downgrade restores the previous class automatically if no other tests or requirements are outstanding.
  • The PDF is a dynamic XFA form that opens only in a current Adobe Reader; other viewers show a placeholder page instead of the form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

How often do Ontario commercial drivers need a medical report?

Every five years under age 46, every three years from 46 to 64 and every year from 65, for class A to F drivers. MTO mails the form 90 days before it is due to the address on the driving record, and class D holders have had to submit regular reports since July 1, 2018.

Is the Ontario commercial driver medical covered by OHIP?

No. The form states the cost of completion by the health care provider is the patient's responsibility and is not a benefit of OHIP or the Ministry of Transportation. ontario.ca tells drivers they pay the examination fee and suggests checking the cost with the practitioner first.

What hearing standard applies to Ontario commercial drivers?

For classes B, C, E and F, ontario.ca requires perceiving a forced whisper at 1.5 metres in the better ear, with or without a hearing aid, or a loss of no more than 40 decibels at 500, 1000 and 2000 hertz on audiometry. The form's hearing section records the whisper test and hearing aid use.
