Ontario School Vaccine Medical Exemption (4895-64E)

Ontario's Ministry of Health uses the Statement of Medical Exemption (4895-64E) when a pupil needs a medical exemption from one or more vaccines required for school attendance under the Immunization of School Pupils Act. For each disease, the physician or nurse practitioner indicates immunity, through prior disease or laboratory confirmation, or a contraindication detrimental to health, and whether the exemption is permanent or temporary.

Who completes it

A physician or registered nurse in the extended class (nurse practitioner) completes the Section 2 declaration and signs Section 3 with their business address. Section 1 records the pupil's details, school name and class or grade.

Good to know

  • The diseases listed are diphtheria, tetanus, pertussis, polio, meningococcal disease, measles, mumps, rubella and varicella, each with its own reason and length of exemption.
  • For varicella, a clinical diagnosis of prior varicella or herpes zoster disease is acceptable evidence of immunity.
  • Temporary exemptions need from and to dates, and free-text boxes define the evidence of immunity and explain contraindications detrimental to health.
  • Licensed child care uses a different form, the Ministry of Education's 3041E under the Child Care and Early Years Act, 2014, which also lists Hib.
