Sports physical and preparticipation exam forms for every state

State athletic association physicals, concussion and skin lesion releases, and NCAA, USADA, Special Olympics and dive medical forms, with who signs each one.

Most state high school associations build their sports physical on the Preparticipation Evaluation (PPE) forms from the American Academy of Pediatrics and partner societies. The PPE Physical Examination Form records the exam, and the Medical Eligibility Form clears the athlete for all, some or no sports. Texas UIL, Kansas KSHSAA, Louisiana LHSAA, Montana MHSA and Michigan MHSAA each publish their own version. This page lists 71 forms, including concussion return-to-play releases, wrestling skin lesion releases and NCAA, USADA and Special Olympics forms.

In many states only the Medical Eligibility Form goes to the school, and the history and exam stay in the medical record. Signer rules differ: Alabama's Form 5 and Wyoming's A-7/A-8 have no NP or PA line, Kansas requires an MD or DO to release a concussed athlete, and New Jersey examiners must first complete the NJDOE cardiac assessment module. 25 forms have a Spanish version, often for the history pages only. Vero Documents fills PDF forms from the visit note and the chart, and you review and sign.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign a sports physical form?

It depends on the state association. The AAP PPE exam form has a signature line for an MD, DO, NP or PA. Alabama's Form 5 requires an MD or DO, and Wyoming's A-7/A-8 allows an MD, DO or DC only. Arizona also accepts naturopathic physicians and certified chiropractic sports physicians. Since March 10, 2025, Utah requires the examining provider to be board certified, and New Jersey requires the NJDOE cardiac assessment module.

How long is a sports physical valid?

Most count for one school year, with a start date for next year's exams. Michigan counts physicals given on or after April 15, Indiana on or after April 1, and Arizona on or after March 1. Florida's EL2 is valid for 365 days. Alaska accepts an exam within the previous 18 months, Montana exams on or after May 1 cover the next two school years, and Wisconsin requires a physical every other year.

Who can clear a student athlete to return to play after a concussion?

State law sets the list. Kansas requires a written release from an MD or DO, with no NP or PA option. Michigan accepts an MD, DO, PA or NP, but the clearance must be unconditional. Illinois also allows certified athletic trainers, and North Carolina's Gfeller-Waller Form D allows physicians, athletic trainers, PAs, NPs and neuropsychologists. Louisiana's Return to Competition Form requires the provider's Louisiana medical license number.

What is the Florida EL1 ECG screening form?

The EL1 confirms that a student-athlete completed a qualifying ECG done on or after July 1, 2024. From July 1, 2026 it is required for incoming 9th graders and new 10th to 12th grade athletes in FHSAA sports, and it is turned in with the EL2 physical. If the ECG or the physical needs more evaluation, the follow-up clinician uses the EL1/2s supplemental form. A medical doctor can exempt a student on the ME1.
