Louisiana LHSAA Medical History and Physical Exam

LHSAA requires this medical history evaluation and physical exam every year for Louisiana high school athletes, kept on file at the school and subject to inspection by the association's Rules Compliance Team. Page 1 holds the family and athlete history and a parents' waiver; page 2 is the provider's limited screening exam and medical eligibility decision.

Who completes it

An MD, DO, APRN or PA completes the page 2 exam and eligibility section and signs. The parent completes the page 1 history and waiver and answers four consent questions.

Submitting and payment

How to submit it
The completed form is returned to the school, which keeps it on file for inspection.
Who pays
LHSAA sets no fee; if the screening is given without expectation of payment, the waiver cites La. R.S. 9:2798, limiting claims against a volunteer provider.

Good to know

  • The physical expires 13 months from the date the MD, DO, APRN or PA signs and dates it.
  • The eligibility choices follow the standard categories, and the form states the recommendation comes from a limited screening.
  • Family history asks about relatives under 50 with heart disease, sudden death, stroke, high blood pressure, diabetes, sickle cell trait and other conditions.
  • LHSAA posts one-page and two-page versions of the form, plus a version with the history and waiver page in Spanish.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
