KSHSAA requires an annual history and physical for athletes and cheerleaders, taken no earlier than April 1 before the school year it covers. The packet adapts pages 1 to 4 from the 2019 preparticipation physical evaluation: the family completes the history, and the provider completes the exam on page 3 and signs the Medical Eligibility Form on page 4.
The health care provider reviews the history with the student and parent, completes the page 3 exam and the page 4 eligibility section, and signs as MD, DO, DC, PA-C or APRN. The family completes pages 1 and 2 and signs pages 4 and 5.

KSHSAA Pre-Participation Physical Evaluation
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

The annual history and physical can be taken no earlier than April 1 preceding the school year it applies to. KSHSAA recommends finishing it at least one month before the first practice to allow time to correct deficiencies and start conditioning recommendations.
The signature line on page 4 lists MD, DO, DC, PA-C and APRN. The provider marks one of the eligibility outcomes, adds comments and recommendations, and enters the date of examination, address and phone.
Yes. KSHSAA posts the pre-participation physical evaluation in English and Spanish on its health and safety page; the current English packet is revised 9/2025. A parent and student concussion information release form is posted in both languages too.
