Kansas KSHSAA Wrestler Skin Lesion Release

Kansas follows NFHS wrestling rules that require current written documentation from an appropriate health care professional before a wrestler suspected of a communicable skin condition can compete. KSHSAA's form, adapted from the NFHS release and approved by its sports medicine advisory committee in 2018, records the diagnosis, lesion locations, treatment and the earliest date the wrestler may return.

Who completes it

The provider completes the whole form, marking the location and number of lesions on the body diagram, medications, treatment start date and time, an expiration date and the earliest return date, then signs as MD, DO, DC, PA-C or APRN.

Submitting and payment

How to submit it
The coach furnishes the signed form at the weigh-in for the dual meet or tournament.
Who pays
KSHSAA sets no fee for the release and does not say who pays for the visit.

Good to know

  • A first episode of herpes gladiatorum calls for at least 10 days of treatment before wrestling, or 14 days with fever or swollen nodes; recurrences need 120 hours of oral antivirals.
  • Bacterial lesions need at least 72 hours of oral antibiotics, all lesions scabbed and no new lesions in 48 hours; continuing new or draining lesions after 72 hours should raise MRSA concern.
  • A designated on-site meet health care professional may overrule the signing provider's decision for or against participation.
  • Documentation of a non-communicable condition such as eczema, psoriasis or a birthmark is valid for the season, though a secondary infection may require re-evaluation.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Kansas wrestler skin lesion form?

The provider signature line lists MD, DO, DC, PA-C and APRN. The provider's name must be legible, with office phone and address. KSHSAA and NFHS state they are not responsible for the diagnosis or any action taken in reliance on the form.

Can a wrestler in Kansas cover a contagious lesion to compete?

No. Covering a communicable condition is not acceptable and does not make the wrestler eligible. Once a lesion is considered non-contagious, it may be covered to allow participation; molluscum contagiosum may be covered with a bioclusive dressing and wrestled immediately.

How long is the KSHSAA skin lesion release valid?

The provider sets it: the form has a field for the expiration date for that lesion, noted on the body diagram, along with the earliest date the wrestler may return. Season-long documentation applies only to non-communicable conditions such as eczema, psoriasis or birthmarks.
