Michigan MHSAA Pre-Participation Physical Form

MHSAA requires a current-year physical before a Michigan student takes part in school athletics, meaning one given on or after April 15 of the previous school year. Its two-page form has the medical history and the examiner's physical examination and medical clearance on the front, with consent, insurance and emergency treatment signatures on the back.

Who completes it

An MD, DO, PA or NP completes the physical examination and medical clearance section, checks a credential box and signs. The parent, guardian or 18-year-old student completes the history, emergency information and the four signatures on page 2.

Submitting and payment

How to submit it
The examiner returns the completed form directly to the patient, who turns it in to the school.
Who pays
MHSAA sets no fee for the physical and does not say who pays.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What counts as a current-year physical in Michigan?

MHSAA defines a current-year physical as one given on or after April 15 of the previous school year. The rule is printed on both pages of the form, so an exam in late April or May can cover the following school year.

Who can sign the MHSAA physical form?

The physical examination and medical clearance section is completed by an MD, DO, PA or NP, who checks one of those boxes beside the signature. The examiner returns the form directly to the patient rather than to the school.

Is the Michigan physical form available in other languages?

Yes. MHSAA posts the two-page physical form in English, Spanish and Arabic on its physical exam and medical history forms page. MHSAA no longer uses its separate health questionnaire, so the two-page form is the whole requirement.
