Montana MHSA Preparticipation Physical Exam Form

MHSA Handbook Article II, Section 3 requires a physical exam before a Montana student's first practice. This confidential MHSA form, updated 4/26 for 2026-27, is the only form accepted: the family completes the history, including a PHQ-4 screen, and the provider completes the physical examination and clearance page and signs to clear the student.

Who completes it

A licensed medical professional acting within the scope of their practice reviews the history with the family, performs the exam, completes the clearance and signs. The student and parent complete and sign the history and permission sections.

  • In off years, an interim history form is submitted to the school before the first practice.
  • All 9th graders need a physical after May 1 of the year they enter high school, even if they had one in 8th grade.
  • MHSA advises schools that coaches should not see the full form and that records are typically kept 7 years after graduation.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
