Michigan MHSAA Return to Activity Concussion Form

When a Michigan athlete is removed from activity for concussion symptoms and not returned that day, MHSAA rules require unconditional written clearance from an MD, DO, PA or NP and consent from the student and parent before return. This form captures both, Section 1 for the medical examiner and Section 2 for the family.

Who completes it

An MD, DO, physician assistant or nurse practitioner signs Section 1, granting return to unrestricted activity, and prints their name. The student, and a parent or guardian if the student is under 18, sign Section 2.

Submitting and payment

How to submit it
The school keeps the form on file and emails or faxes it to MHSAA; it is kept for seven years after the student's graduation.
Who pays
MHSAA provides concussion care insurance that pays accident medical expenses for in-season suspected concussions, including deductibles and co-pays left by other policies, up to its limits.

Good to know

  • Clearance must be unconditional: approval to begin a return-to-activity progression, or return pending its completion, is not sufficient.
  • The student may not return to practice or competition on the same day as the injury, even with a signed clearance.
  • Schools, districts and leagues may add stricter requirements, and a school or health facility may use a locally created clearance form if it complies with MHSAA rules.
  • Schools separately report every concussion event where a student is withheld from activity online through MHSAA.com, apart from this form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What does unconditional clearance mean on the MHSAA form?

The medical examiner must approve the student's return to unrestricted activity. MHSAA states it is not enough to approve the student to begin a return-to-activity progression, or to authorize return pending completion of one, so the signature should come after any progression is finished.

Who can sign the Michigan Return to Activity form?

An MD, DO, physician assistant or nurse practitioner, who circles the credential next to the signature in Section 1 and prints their name. The student, and a parent or guardian for students under 18, then sign the Section 2 consent.

Where does the MHSAA Return to Activity form go?

The school keeps it on file for seven years after the student's high school graduation and sends a copy to MHSAA by email or fax. The school also records the MHSAA head injury report ID in the school-use section.
