Louisiana LHSAA Return to Competition Form

LHSAA requires a written statement from a health care provider before an athlete returns to competition after a concussion or head injury, and an athlete found to have a concussion cannot continue practice or competition that day. This one-page form is that statement: the attending provider permits the athlete to return on a stated date in a stated sport.

Who completes it

The attending health care provider, a physician as defined in R.S. 37:1262(2), a licensed nurse practitioner or a licensed physician assistant, completes the form with a Louisiana medical license number and signs.

Submitting and payment

How to submit it
The completed form is kept by the school and by the health care provider; LHSAA does not keep the record.
Who pays
LHSAA sets no fee and does not say who pays for the provider's evaluation.

Good to know

  • If a health care provider recommends that an athlete not continue, that recommendation may not be overruled.
  • Athletes also need written clearance from their health care provider to complete the graduated return-to-play progression.
  • The form must be completed in its entirety: athlete, school, sport, date of concussion, date to return, provider name and Louisiana license.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the LHSAA Return to Competition Form?

The attending health care provider, defined on the form as a physician under R.S. 37:1262(2), a licensed nurse practitioner or a licensed physician assistant. The provider enters a Louisiana medical license number, signs and dates the form.

Does LHSAA keep the Return to Competition Form?

No. The form should be maintained by the school and the health care provider, and LHSAA states it does not maintain this record. It must be completed in its entirety, including the date of concussion and the date the athlete may return.

Can a coach overrule a provider's decision to hold an athlete out?
