Minnesota MSHSL Sports Qualifying Physical Exam

MSHSL requires a sports qualifying physical for Minnesota high school athletes. The 2026-2027 form has five pages: the Medical Eligibility Form the school receives, a history form, the physical exam form, a supplement for athletes with disabilities and an addendum for Adapted Athletics PI Division eligibility, all based on the 2019 preparticipation physical evaluation.

Who completes it

The examining provider completes the exam and signs the page 1 Medical Eligibility Form, choosing one of four eligibility outcomes. The student and parent complete and sign the history before the appointment.

Submitting and payment

How to submit it
A copy of page 1 goes back to the school with the student; the provider keeps pages 2 to 5 on file.
Who pays
MSHSL sets no fee for the physical and does not say who pays.

Good to know

  • The form is valid for 3 calendar years from the exam date with a normal Annual Health Questionnaire in years 2 and 3.
  • Page 1 classifies sports by contact level and by static and dynamic intensity, so the provider can cross out activities a student may not do.
  • The exam page records blood pressure in both arms, arm span and hearing, with optional BMI, body fat and Tanner staging, and carries code Z02.5.
  • MSHSL posts the form in English, Spanish, Somali and Hmong on its health and safety page and its provider resources page.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Which pages of the MSHSL physical go to the school?
