Illinois IHSA Post-Concussion Consent Form (RTP/RTL)

Illinois's Youth Sports Concussion Safety Act requires schools to set Return-to-Play and Return-to-Learn protocols that a student completes before fully returning after a concussion. On this IHSA form the student and parent consent to those protocols and to sharing the treating provider's written statement, which must be attached and say it is safe to return to play and learn.

Who completes it

The treating physician, advanced practice nurse, physician assistant, or athletic trainer working under a physician's supervision provides the written statement attached to the form. The student and parent sign the consent, and the school records clearance dates.

Submitting and payment

How to submit it
The signed consent and the provider's written statement are kept by the school, which records the dates the student is cleared for return to learn and return to play.
Who pays
IHSA sets no fee and does not say who pays for the provider's evaluation.

Good to know

  • The form has no provider signature line; the school checks that the treating provider's written statement is included before marking clearance.
  • For same-day return after removal from a contest, IHSA allows only certified athletic trainers, APNs, PAs and physicians licensed in all branches of medicine in Illinois to clear the athlete.
  • Under the 2015 Youth Sports Concussion Safety Act, schools must have a concussion oversight team and their own RTP and RTL protocols.
  • By signing, the parent consents under HIPAA to disclosure of the provider's statement and any RTP and RTL recommendations to appropriate persons.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can clear an Illinois student to return after a concussion?

The form accepts a written statement from the treating physician, advanced practice nurse, physician assistant, or an athletic trainer working under a physician's supervision. For a same-day return after removal from a contest, IHSA limits clearance to certified athletic trainers, APNs, PAs and Illinois physicians licensed in all branches.

Does the provider sign the IHSA post-concussion consent form?

No. The student and parent sign the consent. The provider supplies a separate written statement that the school attaches; the school-use box confirms the statement is included before the school records the return-to-learn and return-to-play clearance dates.

What should the provider's written statement say?

It should indicate that, in the provider's professional judgment, it is safe for the student to return to play and return to learn. Any return-to-play and return-to-learn recommendations from the provider can be shared with appropriate persons under the parent's consent.
