Indiana IHSAA Pre-Participation Physical Evaluation

IHSAA Rule 3-10 requires an Indiana student-athlete to have a physical exam between April 1 and the first practice, with a completed Consent and Release Certificate filed with the principal. The five-page IHSAA form, based on the 2019 preparticipation physical evaluation, combines the history, the physical exam, a summary of eligibility rules and the consent and release certificate.

Who completes it

A physician (MD or DO), nurse practitioner or physician assistant licensed in Indiana completes the page 3 physical examination and signs it with a license number. The student and parents complete the history and sign pages 2 and 5.

Submitting and payment

How to submit it
The examination form and the signed Consent and Release Certificate go to the member school and are filed in the principal's office; the history form stays with the provider.
Who pays
IHSAA sets no fee for the physical and does not say who pays.

Good to know

  • The signature must be handwritten. Signature stamps and pre-signed or pre-stamped forms are not accepted, and the form may not be altered.
  • Sign only after reviewing the history, performing the exam and completing the form in its entirety, with the license number on page 3.
  • An exam on or after April 1 is valid for the following school year, and a separate form is required for each school year.
  • Rule 3-11 separately requires written verification from a physician before a student absent five or more days for illness or injury returns.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
