SDHSAA Wrestling Skin Lesion Physician Release (SD)

South Dakota posts the NFHS Medical Release Form for Wrestler to Participate with Skin Lesion(s) under Wrestling on its athletics forms page. A wrestler suspected of a communicable skin condition needs this written release at weigh-in, documenting the diagnosis, treatment start and the earliest date the wrestler may return.

Who completes it

The examining provider completes the whole release: diagnosis, lesion location and number on the body diagram, medications, treatment start date and time, form expiration date and earliest return date, then signs with office phone and address.

Submitting and payment

How to submit it
The coach presents the completed release at weigh-in for the dual meet or tournament; a designated on-site meet health care professional may examine the wrestler instead and can overrule the release.
Who pays
SDHSAA sets no fee for the release and the form does not say who pays for the visit.

Good to know

  • The minimum treatment guidelines call for 72 hours of oral antibiotics for impetigo or boils, with no new lesions in 48 hours and all lesions scabbed over.
  • A first episode of herpes gladiatorum means at least 10 days of treatment before competing, extended to 14 days with fever or swollen lymph nodes.
  • Covering a contagious lesion does not make a wrestler eligible; once a lesion is non-contagious, it may be covered to wrestle.
  • The copy SDHSAA posts is the NFHS Sports Medicine Advisory Committee version revised and approved in April 2018, with a first page explaining the form's goals.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the South Dakota wrestling skin release?

SDHSAA lists it as a physician release. The NFHS model form itself leaves it to each state association to decide which appropriate health care professional may sign, and its signature block asks only for the provider's signature, legible name, office phone and office address.

How long is a wrestling skin release valid?

The provider writes a form expiration date for the specific lesion and notes it on the body diagram. Separately, NFHS Rule 4-2-5 lets documentation of a non-communicable condition such as eczema, psoriasis or a birthmark remain valid for the whole season.

What treatment is needed for ringworm before wrestling?

The form's minimum guidelines call for 72 hours of oral or topical treatment for tinea on the skin and 14 days of oral treatment for tinea on the scalp. NFHS presents these as suggested minimums, not as a standard of care.
