SDHSAA Physical Exam Form (South Dakota Sports PPE)

South Dakota's SDHSAA participation packet for 2026-27 includes the Preparticipation Physical Exam Form, the record of the examination in years an exam is required. Each school board sets how often exams happen, at least once every three years, so the packet also carries health history, interim, consent, HIPAA, concussion and cardiac pages.

Who completes it

Parents and students complete the health history (exam years) or interim form (off years) plus the consent pages. A doctor of medicine, osteopathy or chiropractic, a licensed physician assistant or a licensed nurse practitioner completes the exam form, marks a participation recommendation and signs.

Submitting and payment

How to submit it
The forms are returned to the school, which keeps them on file and must store student health information under HIPAA and FERPA rules.
Who pays
SDHSAA sets no fee for the exam and the packet does not say who pays for it.

Good to know

  • Member schools may accept physical exams completed after April 1 of the previous school year for the ensuing school year.
  • Stamping a clinic or association name is not acceptable; each form must be signed by an authorized medical professional.
  • The health history must be present at the exam, and the exam form records whether the visit is the annual, biennial or triennial exam.
  • Physician reminders on the form include stress, safety at home, tobacco and vaping, anabolic steroids, weight or performance supplements, and seatbelt or helmet use.

Frequently asked questions

Can a chiropractor do a South Dakota high school sports physical?

How often does South Dakota require a sports physical?

Each school board decides. SDHSAA guidelines say physicals should be completed at least once every three years, and districts may require them annually, biennially or triennially. In years without an exam, students complete the SDHSAA Interim Pre-Participation Form instead.
