VPA Wrestling Skin Infection Form (Vermont NFHS)

The Vermont Principals' Association posts this NFHS form for wrestlers with a skin infection on its sports medicine page. When a referee or coach suspects a communicable skin condition, the wrestler needs a completed release from a health care professional at weigh-in stating the condition is not communicable.

Who completes it

The examining provider completes the whole one-page form: legible name, diagnosis, date of exam, lesion location and number on the diagram, medications, treatment start date and time, form expiration date, earliest return date, signature and office details.

Submitting and payment

How to submit it
The coach presents the completed form at weigh-in for the dual meet or tournament; VPA hosts the form as a Google Drive file linked from its sports medicine page.
Who pays
VPA sets no fee and the form does not say who pays for the visit.

Good to know

  • The VPA copy is the NFHS SMAC version approved in April 2022; NFHS reviews the form yearly and has since issued newer versions.
  • Tinea requires 72 hours of oral or topical treatment on skin and 14 days of oral treatment on the scalp before return.
  • Molluscum contagiosum treated with curettage and hyfrecator may be covered with a bio-occlusive dressing, and the wrestler may compete immediately.
  • Non-contagious conditions such as eczema or psoriasis need no treatment before return, and documentation of them is valid for the season.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who signs the Vermont wrestling skin infection form?

The form has a provider signature line with a legible printed name, office phone and office address; it does not list credentials. NFHS leaves it to each state association to decide which appropriate health care professionals may sign the release.

Can a wrestler compete with a covered skin infection in Vermont?

Only if the lesion is non-contagious. The form quotes NFHS Rule 4-2-3: covering a communicable condition is not acceptable and does not make the wrestler eligible. Once a lesion is considered non-contagious, it may be covered to allow participation.

Does Vermont have a statewide sports physical form?

No. VPA publishes no statewide physical form, so schools use their own or provider forms. Its sports medicine page does post concussion return-to-play sign-off sheets and a recommended return-to-learn protocol alongside this wrestling skin form.
