NSAA Pre-Participation Physical Form (Nebraska PPE)

NSAA Bylaw 3.4.1 requires every Nebraska student who plans to compete in interschool athletics to give the superintendent or principal a physician's certification of fitness once each year, before participating. NSAA's recommended packet is the PPE 5th edition set: history form, athletes with disabilities supplement, physical examination form and Medical Eligibility Form.

Who completes it

The student and parent complete and sign the history form before the appointment. The examining provider completes the physical examination and Medical Eligibility Form, and a parent may sign a release on the exam page allowing the history and exam results to go to the school.

Submitting and payment

How to submit it
The signed certification is returned to the school's superintendent or principal once each year before the student participates.
Who pays
NSAA sets no fee for the physical and the forms do not say who pays for it.

Good to know

  • NSAA accepts exams from providers licensed to diagnose and treat the areas the form covers, acting within scope, and its 2026 guidance includes Doctors of Chiropractic who meet Bylaw 3.4.1.1.
  • The printed signature lines read MD, DO, NP or PA, so chiropractors rely on the separate NSAA guidance on preparticipation physicals by chiropractors.
  • The physical examination page lists physician reminders on stress, mood, safety at home, tobacco, alcohol and drug use to consider asking during the visit.
  • The history form asks for sex assigned at birth and gender identity, and the Spanish file NSAA posts is the history form only.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Can a chiropractor sign the Nebraska NSAA physical?

Yes, if the chiropractor meets NSAA Bylaw 3.4.1.1. NSAA guidance updated in 2026 says it accepts exams from providers appropriately licensed to diagnose, treat or recommend treatment in the areas the form covers, within the limits Nebraska law sets, and names Doctors of Chiropractic among them.

How often does Nebraska require a sports physical?

Once each year. Bylaw 3.4.1 requires each student who intends to participate in interschool athletic competition to provide the superintendent or principal with a physician's certification of physical fitness annually and before participation begins.

Is the NSAA physical form available in Spanish?

Partly. NSAA posts a Spanish version of the history form, Evaluación Física de Preparticipación-Forma Historial, so families can complete their part in Spanish. The examination and eligibility pages that the provider completes are posted in English.
