TSSAA Preparticipation Physical Form (Tennessee)

TSSAA/TMSAA Bylaws Article II, Section 10 bar a Tennessee student from practice or contests until a preparticipation medical evaluation is on file with the principal. The nine-page PDF opens with a letter from TSSAA, the Tennessee AAP chapter and TennCare encouraging PPEs at well visits, then the PPE history, exam and eligibility pages and parental consent.

Who completes it

A doctor of medicine, osteopathic physician, physician assistant or certified nurse practitioner examines the student and signs the exam and eligibility pages. The student and parent complete the history and sign the emergency contact and legal/parent consent page.

Submitting and payment

How to submit it
The signed form is filed with the school principal before practice; the principal may instead accept a signed provider statement covering every element of the form.
Who pays
TSSAA sets no fee for the physical, and its cover letter notes that well visits are free for students enrolled in TennCare.

Good to know

  • The physical must be given no earlier than April 15, and the provider must state the student is physically fit for interscholastic athletics.
  • Parents may decline the exam on grounds of sincerely held beliefs or practices by filing a signed statement with the principal, affirmed under penalty of perjury.
  • The consent authorizes TSSAA, its physicians, athletic trainers and EMTs to render aid and treatment, and consents to screening and recording of history findings.
  • A Spanish version of the consent to participate and receive medical care follows the English consent in the PDF.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign a Tennessee TSSAA sports physical?

Bylaws Article II, Section 10 list a doctor of medicine, osteopathic physician, physician assistant or certified nurse practitioner. The provider must state that the student passed a physical examination and is physically fit to participate. The eligibility page's signature line reads MD, DO, NP or PA.

When is the earliest date for a TSSAA physical?

The bylaw requires the physical examination to be given not prior to April 15. The form must be on file with the principal before the student takes part in any practice session or athletic contest, together with a signed parental consent certificate.

Can a doctor's letter replace the TSSAA physical form?

Yes. In lieu of the form, the principal may accept a signed statement from the health care provider certifying that the student passed a physical examination covering all elements of the preparticipation medical evaluation form and is physically fit for interscholastic athletics.
