TSSAA Concussion Return to Play Form (Tennessee)

Tennessee's sports concussion law (T.C.A. 68-55-501) keeps a student removed for a suspected concussion out until a licensed health care provider clears return. TSSAA's form records the evaluation, physical education and sports recommendations, and a six-day gradual return-to-play plan adapted from the CDC Acute Concussion Evaluation care plan.

Who completes it

The treating provider fills in the evaluation details, initials the selected recommendations and signs, checking whether they are an MD, DO, clinical neuropsychologist or PA with concussion training. The athlete and family follow the plan but have no signature section.

Submitting and payment

How to submit it
The athlete brings the completed form to the school, which keeps it on file before allowing a return to practice or play.
Who pays
TSSAA sets no fee and the form does not say who pays for the evaluation.

Good to know

  • Sports options include no return yet, gradual return under the school or team health care professional, advancement after a phone call with the treating provider, or full clearance.
  • Each step of the six-day plan is taken only without recurrence of symptoms and without pain medication; if symptoms return, the athlete restarts at the first level.
  • The provider also sets a return-to-office date and time and a return-to-school date on the first page of the form.
  • The plan adapts the CDC care plan and the Tennessee Return to Learn/Return to Play concussion management guidelines.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
