Iowa Athletic Pre-Participation Physical Exam Form

Iowa's boys' and girls' athletic associations share this four-page physical form, an annual exam valid for one calendar year. The student and parent complete the history, including a PHQ-4 screen, before the visit. The provider completes the physical exam on page 3 and the participation eligibility section on page 4, the page families can release to the school.

Who completes it

The medical provider completes the page 3 exam and page 4 participation eligibility and signs. The athlete and parent complete pages 1 and 2, and the parent signs the page 4 consent and adds shared emergency information.

Submitting and payment

How to submit it
The family returns the form to the school, choosing on page 4 whether to release the full form or only page 4.
Who pays
The associations set no fee for the physical and do not say who pays.

Good to know

  • Parents can release the full form or only page 4 to the school, and agree to tell the school if the student's health changes in a way that affects participation.
  • The PHQ-4 on page 1 is scored by the provider; a sum of 3 or more on either two-question subscale is positive for screening.
  • The heart section suggests considering ECG, echocardiography or cardiology referral for abnormal cardiac history or exam findings.
  • Page 4 asks for known health conditions that could affect activities or be important for athlete care, in addition to the eligibility decision.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
