Florida FHSAA ME1 ECG Screening Medical Exception

Florida's Second Chance Act added an ECG screening requirement for high school student-athletes. The ME1 Certificate of Medical Exception lets a licensed physician document a medical reason a student should be excepted from that requirement. The physician states the reason, and the school keeps the certificate for as long as the student participates in high school sports.

Who completes it

A physician licensed under Chapter 458 or 459, Florida Statutes, completes the exception and signs with credentials and license number, certifying training in the International Criteria for Interpreting ECGs in Student-Athletes. The student and parent fill in the student information.

Submitting and payment

How to submit it
The certificate is submitted to the student's school, which retains it for the duration of the student's participation in high school sports.
Who pays
FHSAA sets no fee for the certificate and does not say who pays.

Good to know

  • FHSAA released the ME1 in 2026 with the EL1 ECG Screening Form, required from July 1, 2026 for incoming 9th graders and grade 10 to 12 students new to FHSAA sports.
  • Returning athletes are not required to get an ECG, though FHSAA strongly encourages it.
  • Chapters 458 and 459 cover medical and osteopathic physicians, so the ME1 is a physician's form; FHSAA describes it as for use by medical doctors.
  • The form is revised 1/26 and is listed with the other eligibility forms under the Parents tab on the FHSAA website.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the FHSAA ME1 medical exception?

A physician licensed under Chapter 458 or 459 of the Florida Statutes, meaning an MD or DO. In the licensed practitioner section, the signer certifies training in the International Criteria for Interpreting ECGs in Student-Athletes and adds credentials and license number.

How long does the school keep the ME1?

The form says the school must retain it for the duration of the student's participation in high school sports. The exception is documented once rather than renewed each year, and the student information section identifies the student, school, grade and student ID.

Which Florida students need an ECG for sports?

FHSAA's announcement says the ECG requirement applies to all incoming 9th grade student-athletes for 2026-27 and to students in grades 10 to 12 who have never played an FHSAA-sanctioned sport. An ECG performed on or after July 1, 2024 satisfies it.
