USADA TUE Application: Therapeutic Use Exemption

An athlete under USADA's anti-doping jurisdiction who needs a substance or method on the WADA Prohibited List applies for a Therapeutic Use Exemption with this five-page form. The athlete completes sections 1 to 3, and the treating physician completes sections 4 to 6 with diagnosis, medication details and a signed declaration.

Who completes it

The athlete or a designated representative completes sections 1 to 3 (athlete information, competition level and schedule, TUE type) and signs the privacy declaration. The treating provider completes the diagnosis with ICD or DSM code, the medication table and the declaration, and a supervising MD or DO signs when applicable.

Submitting and payment

How to submit it
The athlete submits the completed application and medical records to USADA through the secure online portal on the TUE apply page; photos or JPEGs of the form are not accepted.
Who pays
The application sets no fee and does not say who pays for the physician's documentation.

Good to know

  • Many TUEs are returned or denied because there is no documentation of failed trials of permitted alternatives or of why an alternative is not feasible.
  • The medication table must be filled in completely (substance, dose, route, frequency, duration); referencing attached records is not adequate.
  • Standard requests take up to 21 calendar days; expedited and retroactive categories exist, including emergency or urgent treatment under ISTUE Article 4.1(a).
  • USADA condition checklists (ADHD, asthma, diabetes and others) and WADA checklists list the records the TUE committee needs to decide without seeing the athlete.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What medical records go with a USADA TUE application?

A complete, legible application, a comprehensive medical history of the condition, copies of relevant clinical evidence such as visit notes, labs and imaging, and a physician statement explaining why the prohibited substance is needed and why permitted alternatives are not appropriate.

How long does a USADA TUE take?

The standard TUE type is listed as taking 21 calendar days to process. An expedited option exists for athletes competing within 21 days, but USADA gives no guarantee of a decision before the competition. Applications without enough medical documentation may be returned without review.

Does the USADA TUE apply to high school athletes?

No. It applies to athletes subject to USADA's anti-doping rules, such as Olympic and Paralympic movement athletes, not school sports. The form does ask whether the athlete is currently an NCAA student-athlete and about national governing body and international federation membership.
