NCAA ADHD Medical Exception Documentation Form

After an NCAA drug test comes back positive for a stimulant, a school requesting a medical exception submits this form with the athlete's ADHD documentation to the NCAA-designated drug-testing agency. The physician block is short; the attached clinical evaluation, medication history and follow-up plan carry most of the weight.

Who completes it

The college or university fills in the school, representative, athlete and medication fields. The athlete's current treating physician prints name, specialty and office address and signs, attaching an evaluation summary by a physician, psychologist or neuropsychologist, psychiatrist or psychiatric nurse practitioner trained in ADHD.

Submitting and payment

How to submit it
The school submits the form and documentation to the NCAA-designated drug-testing agency, as described in Section 8.0 of the NCAA Drug-Testing Manual.
Who pays
The NCAA form sets no fee and does not say who pays for the evaluation or documentation.

Frequently asked questions

What documentation does the NCAA require for an ADHD medical exception?

A comprehensive clinical evaluation summary meeting DSM-5 criteria, evaluation details and history, mandatory collateral from a secondary source, each stimulant's name, dose, frequency and dosing changes, non-banned alternatives tried, the treatment start date, and a monitoring plan with follow-up visits and assessments.

Who can evaluate ADHD for the NCAA form?

The evaluation summary may come from a physician, psychologist or neuropsychologist, psychiatrist or psychiatric nurse practitioner with training in diagnosing or treating ADHD. The form itself is signed by the athlete's current treating physician, who also lists their specialty.

Who sends the ADHD form to the NCAA?

The college or university. A school representative fills in the top section with their name, title and contact details and submits the request to the NCAA-designated drug-testing agency under Section 8.0 of the Drug-Testing Manual after a positive stimulant test.
