NFHS Wrestling Skin Lesion Release Form 2026-27

The NFHS Sports Medicine Advisory Committee publishes this model release so an appropriate health care professional can document that a wrestler's skin lesion is not communicable. State associations adopt or adapt it, and NFHS Rule 4-2-3 requires current written documentation at weigh-in when a referee or coach suspects a communicable skin condition.

Who completes it

The examining health care professional completes the release page: wrestler's name, diagnosis, lesion location and number on front and back diagrams, medications, treatment start date and time, form expiration date, earliest return date and signature. Each state association decides which professionals may sign.

Submitting and payment

How to submit it
The coach furnishes the completed release at weigh-in for the dual meet or tournament, unless a designated on-site meet health care professional examines the wrestler immediately before or after weigh-in.
Who pays
NFHS sets no fee and the form does not say who pays for the visit.

Good to know

  • The 2026-27 version was approved by the NFHS SMAC in May 2026; NFHS reviews the form yearly, so state copies may carry older approval dates.
  • Minimum treatment guidelines include 24 hours for scabies, head lice or conjunctivitis, and 120 hours of oral antiviral therapy for recurrent herpes outbreaks.
  • NFHS suggests marking lesions in non-black ink and recording their number, which helps officials spot new lesions after the visit.
  • Under Rule 4-2-4, a designated on-site meet health care professional may overrule the diagnosis of the professional who signed the release.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Who can sign the NFHS skin lesion form?

NFHS leaves that to each state association, which determines which appropriate health care professional can sign off. Depending on the state, that may include MDs and DOs, physician assistants or nurse practitioners, so the state association's version of the form is the one to follow.

Can a wrestler cover a contagious lesion and compete?

No. NFHS Rule 4-2-3 states that covering a communicable condition is not acceptable and does not make the wrestler eligible. After adequate treatment, once the lesion is considered non-contagious, it may be covered to allow participation.

How long must a wrestler with herpes gladiatorum sit out?

For a first episode, the form's guideline is treatment and no competition for at least 10 days, extended to 14 days if fever or swollen lymph nodes are present. All lesions must be scabbed over with no new lesions in the preceding 72 hours.
