Diver Medical Form 10346: Scuba Physician Evaluation

Recreational scuba and freediving students complete the Diver Medical Participant Questionnaire before training, and certain yes answers mean a physician must evaluate them first. The third page, the Medical Examiner's Evaluation Form, records whether the examiner finds conditions incompatible with diving, and the diver takes the signed page back to the instructor or dive facility.

Who completes it

The participant answers ten questions and any follow-up boxes, then signs (a parent or guardian signs for a minor). A certified medical doctor or other legally certified medical provider completes the evaluation page: approved or not approved, signature, credentials, clinic details and an optional stamp.

Submitting and payment

How to submit it
The diver brings all three pages to the medical examiner and returns the signed evaluation to the dive school or instructor named on the questionnaire.
Who pays
The form sets no fee and does not say who pays for the medical evaluation.

Good to know

  • A yes to question 3 (exercise tolerance), 5 (recent surgery), 10 (prescription medications) or any item in boxes A to G on page 2 requires a physician evaluation.
  • The form warns that any additional commentary written on the evaluation page could invalidate it for some agencies or organizations.
  • Examiners are directed to the Undersea and Hyperbaric Medical Society website for guidance on specific medical conditions as they relate to diving.
  • The English version is dated 2026-01-01; the questionnaire is translated into about 30 languages, some still on 2021 or 2022 versions.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Diver Medical evaluation form?

The signature line reads certified medical doctor or other legally certified medical provider. The examiner prints their name and clinical degrees or credentials, adds clinic or hospital and contact details, and may stamp the form. The result is a simple approved or not approved.

When does a diver need a doctor's evaluation?

When the participant answers yes to question 3, 5 or 10 on page 1, or to any question in boxes A through G on page 2, such as heart surgery, asthma limiting exercise, recent sinus or eye surgery, seizures or diabetes. Otherwise the participant signs the statement and no evaluation is required.

Is the Diver Medical form available in Spanish?

Yes. UHMS hosts a Spanish version of form 10346 alongside the English one, and the questionnaire is published in roughly 30 languages, including Arabic, Chinese, French, German, Japanese, Korean, Portuguese and Tagalog.
