Kentucky KHSAA WR111 Wrestling Skin Condition Form

In Kentucky, a wrestler suspected of a communicable skin condition needs current written documentation that it is not communicable before competing, and KHSAA states WR111 is the only form a referee will accept for that. The physician records the diagnosis, lesion locations, medications and treatment start date, and checks which NFHS rule the condition falls under.

Who completes it

The examining physician (MD or DO) completes and signs the form. The wrestler's name and high school are entered at the top.

Submitting and payment

How to submit it
The coach furnishes the form at the weigh-in for the dual meet or tournament.
Who pays
KHSAA sets no fee for the form and does not say who pays for the exam.

Good to know

  • The form expires seven days from the date of the provider's examination listed on it.
  • KHSAA tells families to copy and retain the original form for future use.
  • An on-site meet health care provider, generally an MD, DO, PA, ARNP or athletic trainer, may overrule the signing physician on whether the wrestler participates.
  • If the exam finds a contagious skin condition under these rules, no covering of the condition will allow the athlete to wrestle.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

How long is the KHSAA WR111 valid?

Seven days. The form states it expires seven days from the date listed as the provider's examination. NFHS rules separately allow documentation of a non-communicable condition such as a birthmark, psoriasis or eczema to remain valid for the season.

Who can sign the KHSAA WR111?

The provider signature line is marked M.D. or D.O., so the form must be signed by a medical or osteopathic physician who examined the wrestler. The physician adds office phone and address and checks the applicable NFHS rule.

What minimum treatment does WR111 list for herpes gladiatorum?

For a first episode, the wrestler should be treated and kept out of competition for at least 10 days, extended to 14 days with fever or swollen lymph nodes. Recurrent outbreaks need at least 120 hours of oral antivirals, with no new lesions and all lesions scabbed over.
