WHSAA Form A-7/A-8: Wyoming School Physical

Wyoming's WHSAA School Physical Examination Medical Record is the four-page form students complete for activities participation. Form A-7/A-8 pairs the medical history and parent consent for emergency care with the physician's exam record, clearance statement and a student and parent informed consent on the risk of injury.

Who completes it

The athlete and parent answer the history questions and sign the emergency medical consent and informed consent. The physician records the exam, marks cleared, cleared after evaluation or rehabilitation, or not cleared, and signs as MD, DO or DC.

Submitting and payment

How to submit it
The completed form is returned to the school; if the physician checks cleared after evaluation or not cleared, a copy must be sent to the appropriate school district.
Who pays
WHSAA sets no fee and the form does not say who pays for the exam.

Good to know

  • The physician's statement must be dated after May 1 to be valid for the upcoming school year.

  • The exam record includes optional body fat percentage, pulses and a genitalia check for males only, with normal findings marked by a check or N.
  • The history asks for the most recent tetanus, measles, hepatitis B and chickenpox immunization dates and whether required school immunizations are up to date.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Wyoming WHSAA physical?

When does a Wyoming sports physical count for the next school year?

The physician's statement must be dated after May 1 to be valid for the upcoming school year. The rule is printed at the top of both the history page and the physical examination page of Form A-7/A-8.

What clearance options are on the WHSAA physical form?

Three: cleared; cleared after completing evaluation or rehabilitation for a stated condition; or not cleared, with the reason. If either of the last two is checked, a copy of the form needs to be sent to the appropriate school district.
