NMAA Pre-Participation Physical Form (New Mexico)

NMAA Bylaw 6.14 requires that this sports physical packet be used for every pre-participation exam in New Mexico. Built on the PPE Monograph 5th edition, the packet bundles emergency information, medical history, physical examination, medical eligibility, consent to treat and concussion awareness forms, and only some of them go to the school.

Who completes it

Parents complete the emergency information, the medical history (jointly with the student), consent to treat and the concussion awareness acknowledgment. The health care provider reviews the history and completes and signs the Physical Examination and Medical Eligibility forms, which carry MD, DO, NP or PA signature lines.

Submitting and payment

How to submit it
The provider returns the Medical Eligibility form to the parent or guardian, who submits it to the school with the emergency, consent and concussion forms; nothing goes to NMAA.
Who pays
NMAA sets no fee for the exam and the packet does not say who pays for it.

Good to know

  • Schools should not collect the Medical History or Physical Examination forms unless the parent or guardian gives written authorization; those pages go back to the family to keep.
  • Providers are asked to review the medical history before the exam and keep a copy of it in their own medical file.
  • The packet reprints AAP cardiac screening guidance: an ECG is the first test when sudden cardiac arrest risk is a concern, read by a provider trained in electrical heart disease.
  • The Spanish file NMAA posts is the medical history form, so Spanish-speaking families can complete their part before the visit.

Fill it with Vero

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Which NMAA physical pages does the school keep?

Schools collect the Emergency Information, Medical Eligibility, Consent to Treat and Concussion Awareness forms. The Medical History and Physical Examination forms stay with the parent or guardian and the provider unless the parent authorizes release in writing. No forms are returned to the New Mexico Activities Association itself.

Who signs the New Mexico sports physical?

A health care provider completes and signs both the Physical Examination and Medical Eligibility forms, whose signature lines are labeled MD, DO, NP or PA. The provider should review the medical history before the evaluation and keep a copy for the medical file.

Is the NMAA physical form available in Spanish?

NMAA posts a Spanish version of the medical history form on its forms page. The rest of the packet, including the examination and eligibility pages the provider completes, is posted in English.
