Alabama AHSAA Form 5 Physical Evaluation

AHSAA Rule 1, Section 14 makes a student in grades 7 to 12 ineligible for interscholastic athletics until a current physician's statement that the student passed a physical exam is on file in the superintendent's or principal's office. Form 5, the revised 2018 Preparticipation Physical Evaluation, is that statement, with the history on page 1 and the exam and clearance on page 2.

Who completes it

The student and parent complete and sign the page 1 history. The examining physician completes the page 2 physical examination and clearance and signs as M.D. or D.O.

Submitting and payment

How to submit it
The signed form is kept on file in the superintendent's or principal's office at the student's school.
Who pays
AHSAA sets no fee for the physical and does not say who pays.

Good to know

  • A physical counts for one calendar year through the end of the month of the exam; an exam on May 5, 2026 satisfies the rule through May 31, 2027.
  • The form must be signed and dated by the attending physician even when a CRNP or PA performed the physical.
  • Only the revised 2018 Form 5 is accepted. AHSAA posts it updated for the 2026-27 school year.
  • Clearance is A (cleared), B (cleared after evaluation or rehabilitation) or C (not cleared for collision, contact or noncontact sports at a given strenuousness), with the reason.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
