OHSAA Concussion Return to Play Authorization (Ohio)

Ohio law, NFHS rules and OHSAA policy bar a student removed for signs of concussion from returning the same day and require written medical authorization before practice or competition resumes. On this one-page OHSAA form the provider certifies they examined the student and states the return-to-play directions the school must follow.

Who completes it

A physician (MD or DO), or another qualified licensed medical provider working in consultation, collaboration or under supervision of an MD or DO, or on their referral, and authorized by the board of education, completes and signs the whole form.

Submitting and payment

How to submit it
The provider completes the form and it is presented to a school administrator before the student resumes practice or competition; the school keeps it indefinitely in the permanent record and the provider should keep a copy.
Who pays
OHSAA sets no fee for the authorization and the form does not say who pays for the exam.

Good to know

  • The provider checks one direction: the Zurich 2012 protocol or an attached one, a protocol under a licensed athletic trainer, no protocol with a return date, sport limits, or other.
  • The form is valid only with all information completed, including the sport, the level (V, JV, 9th or 7-8th) and provider contact details printed or stamped.
  • A licensed athletic trainer, another authorized provider or school administration may seek clarification, and return also depends on conditioning, skill development and mental preparedness.
  • OHSAA publishes a separate Concussion Report Form; this authorization is the document that clears the student to participate again.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the OHSAA return to play form?

An MD or DO, or another qualified licensed medical provider who works in consultation or collaboration with, under the supervision of, or on referral from an MD or DO, and who is authorized by the board of education or other governing board. The provider writes their name and credential at the top.

Can a student return the same day after a suspected concussion in Ohio?

No. The form restates that Ohio law, NFHS rules and OHSAA policy prohibit a student removed for concussion signs, symptoms or behaviors from reentering practice or competition on the same day. Written medical authorization is needed before any later return.

Does the provider have to require a return-to-play protocol?

Not always. The form lets the provider choose a graduated protocol, a protocol under a licensed athletic trainer or other approved provider, or no protocol with a specific return date. Clearance can also be limited to named sports, or the provider can explain other directions.
