OSSAA Physical Evaluation Form (Oklahoma Sports PPE)

No Oklahoma student may represent a school in athletics or marching band until a physical examination and parental consent certificate are on file. OSSAA's form, updated April 2026 for 2026-27, combines the parent consent, a PPE history with PHQ-4 questions and the physical exam with a medical eligibility statement.

Who completes it

A qualified physician, physician assistant or advanced practice nurse covered by professional liability insurance performs the exam and signs by hand. The parent or guardian and the student complete the history and consent pages and sign them by hand as well.

Submitting and payment

How to submit it
The completed form is filed with the school, which retains the history and parent consent pages, before the first day of practice in the student's activity.
Who pays
OSSAA sets no fee and the form does not say who pays for the physical.

Good to know

  • Physicals must be given no earlier than May 1 of the preceding year and before the first practice, and a physical stays valid until the next required physical.
  • The form must be completed in its entirety; pre-signed or pre-stamped forms are rejected, and signature stamps are not accepted for the provider.
  • Schools may use another form only if it captures at least everything requested on the current OSSAA PPE form.
  • Parents sign the consent every year, which also allows identifying information about the student to be disclosed to OSSAA in eligibility investigations.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Oklahoma OSSAA physical form?

A qualified physician, physician's assistant or advanced practice nurse who is covered by professional liability insurance. The signature must be handwritten and dated; OSSAA does not accept signature stamps or forms that were pre-signed or pre-stamped before the exam.

When can an OSSAA physical be done?

No earlier than May 1 of the year preceding participation and before the student's first day of practice in that activity. Once given, the physical is valid until the next required physical, while the parental consent must be signed each year.

Does the OSSAA form cover marching band?

Yes. OSSAA's rule says no student is eligible to represent the school in athletics or marching band until a physical examination and parental consent certificate are on file with the school, so band members use the same form.
