NCHSAA Form C Concussion Evaluation Recommendations

North Carolina's Gfeller-Waller Concussion Awareness Act sets requirements for clearing student-athletes after a concussion. NCHSAA's Form C records a licensed health care provider's recommendations from that day's evaluation: whether the student stays out of school or returns with accommodations, and whether and how the student may start the Return to Play protocol.

Who completes it

A licensed MD, DO, athletic trainer, PA, NP, neuropsychologist or physical therapist completes the form, initials the recommendations selected and signs. A second signature line names anyone to whom aspects of care are delegated.

Submitting and payment

How to submit it
The completed form goes back to the school's athletic trainer or first responder who will monitor the student.
Who pays
NCHSAA sets no fee and does not say who pays for the evaluation.

Good to know

  • NCHSAA strongly recommends a physician's input and signature before full clearance, and says emergency room and urgent care physicians should not make clearance decisions at the first visit.
  • If a first responder monitors the protocol, the supervising provider must review progress through stage 3 before stage 4 begins, and again after stage 5.
  • Return-to-school options are out of school until a date, return for re-evaluation, return with accommodations from the Return to Learn form, or return now without accommodations.
  • Revised June 2026, Form C is posted in English and Spanish with the rest of the Gfeller-Waller forms.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete NCHSAA Form C?

A licensed health care provider: physician (MD or DO), athletic trainer, physician assistant, nurse practitioner, neuropsychologist or physical therapist. NCHSAA strongly encourages expertise and training in concussion management, and strongly recommends physician input before full clearance.

Can an urgent care physician clear a concussed North Carolina athlete?

Form C says emergency room and urgent care physicians should not make clearance decisions at the time of the first visit, because concussions need monitoring for recurring symptoms with cognitive or physical stress. They can still record recommendations such as not cleared and return for evaluation.

What return-to-play options does Form C offer?

Not cleared for sports or PE, light activity with no risk of head trauma, return to the examining provider before clearance, or start the Return to Play protocol under monitoring by a licensed provider or a first responder, with review points before stage 4 and after stage 5.
