NCAA Medical Exceptions Documentation Reporting Form

NCAA student-athletes who need a banned medication use this form in two situations: a medical exception pre-approval (MEPA) before practicing or competing on certain drug classes, or a medical exception request after a positive drug test. The treating physician signs, and the school attaches the diagnostic evaluation, treatment history, dosage and follow-up orders.

Who completes it

A college or university representative completes the school, athlete and medication fields and the continued-use checkbox. The student-athlete's current treating physician prints name, specialty and office address and signs.

Submitting and payment

How to submit it
Schools submit MEPA and continued-use requests through the NCAA Requests/Self-Reports Online (RSRO) system and send post-test medical exception requests to the NCAA-designated drug-testing agency.
Who pays
The NCAA form sets no fee and does not say who pays for the physician's documentation.

Good to know

  • NCAA must approve anabolic agents, hormone and metabolic modulators, peptide hormones, growth factors, related substances and mimetics before the athlete practices or competes on them.
  • Requests for other banned medications must show that all available medical options were tried before the banned substance was prescribed.
  • Every request includes a diagnostic evaluation with supporting lab work, treatment history, medications and dosage, and follow-up orders.
  • Prior approval does not mean a MEPA is automatically renewed; each submission gets a careful review, and the form was last updated June 2026.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

What is an NCAA MEPA?

A medical exception pre-approval. The NCAA requires schools to obtain it, through the RSRO system, before a student-athlete practices or competes while taking anabolic agents, hormone and metabolic modulators, peptide hormones, growth factors, related substances or mimetics. This form and medical records support the request.

Is this NCAA form used for ADHD stimulants?

For stimulant exceptions after a positive test tied to ADHD treatment, the NCAA provides a separate Medical Exception ADHD Documentation Reporting Form listing ADHD-specific documentation. This general form covers pre-approvals and requests for other banned medications.

Does an approved NCAA medical exception renew automatically?

No. The form states that prior approval does not mean a MEPA will be automatically renewed. Continued-use requests are checked on the form and resubmitted through RSRO with an updated diagnostic evaluation, treatment history, medication and dosage, and follow-up orders.
