UHSAA Wrestling Skin Release & Low Body Fat Clearance

Utah wrestling has two physician sign-offs. The skin lesion release, the NFHS model form, documents that a lesion is non-contagious so the wrestler can compete. The Low Body Fat Physician Clearance lets a wrestler measured below 7% body fat (males) or 12% (females) wrestle at that alpha weight if naturally lean.

Who completes it

For the skin release, the examining physician completes diagnosis, lesion diagram, treatment and return dates and signs. For low body fat, a UHSAA assessor enters alpha date, weight and body fat; an MD, DO, PA-C, DC or registered nurse practitioner circles option A or B and signs, followed by parent and coach.

Submitting and payment

How to submit it
The skin release is presented to meet officials at weigh-in; the low body fat clearance is emailed or faxed to UHSAA and posted to the team's alpha master list before the wrestler competes.
Who pays
UHSAA sets no fee for either form and neither says who pays for the visit.

Good to know

  • The low body fat clearance lasts one season and expires at its end, and UHSAA accepts only this form as a physician clearance.
  • The physician evaluates growth, development and weight fluctuations; option B means the wrestler must compete at a weight meeting the 7% or 12% minimum.
  • Skin release treatment minimums include 72 hours of oral antibiotics for bacterial lesions and at least 10 days for a first herpes gladiatorum episode.
  • Body fat is measured by a UHSAA certified assessor using bioelectrical impedance (BIA) under the Utah Weight Monitoring Program.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Utah low body fat wrestling clearance?

A licensed Medical Doctor, Doctor of Osteopathy, Physician's Assistant (PA-C), Chiropractic Physician or Registered Nurse Practitioner. By circling option A, the provider clears the wrestler as naturally below 7% body fat for males or 12% for females at the recorded alpha weight.

How long does the Utah low body fat clearance last?

One season. The clearance expires at the conclusion of the season, and the wrestler cannot compete until the form is returned to UHSAA and the clearance is reflected on the team's alpha master list. Parent and coach signatures are required for it to be complete.

What does the UHSAA skin lesion release require?

The examining provider records the diagnosis, marks the location and number of lesions on the body diagram, lists medications and the treatment start date and time, and gives a form expiration date and the earliest date the wrestler may return. Contagious lesions cannot be covered to compete.
