UHSAA Physical Form A (Utah Pre-Participation Exam)

Utah High School Activities Association members require an annual pre-participation physical, on file with the Disclosure and Consent Document, before a student joins any athletic activity. Form A, updated April 18, 2023, contains instructions, the parent and student consent, a medical history and the physical examination page the provider completes.

Who completes it

The parent and student complete the health history and sign the disclosure and consent document. An MD, DO, PA-C, registered nurse practitioner or Doctor of Chiropractic examines the student, selects one participation recommendation and signs, acknowledging board certification.

Submitting and payment

How to submit it
The entire completed form is returned to school administration, which reviews it and keeps the original in school files.
Who pays
UHSAA sets no fee and the form does not say who pays for the physical.

Good to know

  • Since March 10, 2025, examiners must be board certified in their discipline, for example FNP-BC or FNP-C for NPs, NCCPA for PAs and an ABMS member board for MDs and DOs.
  • The exam must be completed every school year, before any tryout, practice or contest, and not before March 10 of the previous school year.
  • A form that is not fully completed, including the provider's address and phone number, will not be accepted.
  • Chiropractors also mark whether the student takes no prescribed medication or whether they consulted the prescribing physician.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
